Student/Public Incident Alert Form  

Reports of injury, illness, or damages involving a student or member of the public are not processed through the Online Accident Reporting System. In the event of an incident involving a student or member of the public, complete and submit the Student or Public Incident Alert Form to alertrisk@finance.rutgers.edu

Injuries to Students/Public 

If an injury affecting a student or member of the general public as a result of a university-related activity held on or off university premises is witnessed by or reported to a Rutgers employee, the employee must promptly complete the Student or Public Incident Alert Form  and email it to alertrisk@finance.rutgers.edu
